Root Cause Client Assessment

Let’s get to the root of what’s really going on.

Thank you for reaching out. I read every assessment personally before we speak. Please be as honest and detailed as you can — the more I understand about your situation, the more useful our conversation will be. There are no wrong answers here, and everything you share is completely private.

How this works

  • Seven short steps — most people finish in about 15–20 minutes.
  • It adapts to you: the health deep-dive step only shows the areas that match what you select, so you’ll never answer questions that don’t apply.
  • Your progress is shown at the top of every step, and you can go back anytime.
  • Questions marked * are required. Everything else can be left blank if it doesn’t apply.

01

Section 01

About you

This helps me direct you to the right sections of this form and ensures your protocol is built for your specific physiology.

This is genuinely useful clinically — different ethnic backgrounds have different risk profiles for certain conditions and sometimes different optimal lab ranges. Completely optional.

This helps me understand your daily movement, stress load, and energy demands.

02

Section 02

What you're dealing with

This is the most important question — it determines which sections of this form apply to you.

Be specific — e.g. “crushing fatigue by 2pm every day, hair coming out in the shower, and weight that won’t move no matter how clean I eat.”

Mild — I manage okaySeverely affecting my life every day
Based on what you told me in Section 02, these are the areas we’ll go deeper on. Only the sections relevant to you appear below.
12

Section 12

Your gut & digestion

Everyone fills this in — gut health is where every Iron & Aura protocol begins, regardless of your primary condition.

Post-infectious gut dysbiosis and SIBO are extremely common after food poisoning — this is often the starting point of IBS symptoms that were never connected.

13

Section 13

Medications, supplements & blood results

This includes food allergies, medication allergies, and environmental allergies. Important for your safety when building nutrition and supplement plans.

Some clients react to specific supplements — this helps me avoid recommending anything that hasn't worked for you.

These are tests beyond standard GP pathology.

If you have a recent pathology report, attaching it lets me review your actual numbers before we speak. This is optional — you can also just type any values in the box below.

e.g. TSH 3.2, Free T3 4.1, Ferritin 18, Vitamin D 42 nmol/L, Fasting insulin 14 — any numbers are helpful

14

Section 14

Diet, movement & lifestyle

This helps me give you a dietary protocol you can actually follow — not one that requires 3 hours of cooking every day.

Gluten is the single most important dietary question for thyroid, autoimmune, PCOS, gut, and skin conditions. Be honest — there are no wrong answers here.

Dairy is the second most significant dietary trigger across inflammatory, hormonal, and skin conditions.

This is relevant for ALL clients — certain ethnic backgrounds (including South Asian, East Asian, and others) are more predisposed to several of these conditions, sometimes at lower body weights than average.

15

Section 15

Fitness history & training

As a Functional Fitness Coach, understanding your training background and access to equipment helps me design a protocol that works with your hormones — not against them.

Be honest — this is about your real availability, not your ideal. I build your program around your life, not the other way around.

This matters for cortisol and hormonal timing — especially for clients with adrenal or thyroid issues.

This is completely optional. It helps me understand your starting point for both health and fitness goals.

16

Section 16

Stress, sleep & energy

Running on emptyFully energised
Calm and steadyConstantly overwhelmed

Circadian rhythm directly affects cortisol, thyroid, and hormone output — this matters more than most people realise.

Sunlight exposure directly regulates cortisol rhythm, Vitamin D, and melatonin — all critical for hormonal health.

17

Section 17

What you've already tried

e.g. “They said my thyroid is borderline but not treating it yet,” or “just lose weight and take the pill.”

18

Section 18

Your goals

Be specific — not just “feel better.” Regular periods? Hair stops falling? Energy back? Weight shifting? Anxiety gone? Clear skin? Stronger in the gym?

This is the most important question on this form. Take 30 seconds and really think — would you stop cancelling plans? Feel like yourself again? Have energy for your kids? Wear clothes you’ve been avoiding? Get back to training?

19

Section 19

Your readiness

Not sure I'm ready yetI'm done waiting — ready to do what it takes

This includes exercise, meal preparation, and any lifestyle changes.

Your history, something you tried that didn’t work, something your doctor dismissed that you believe matters.

20

Section 20

Your contact details

You've made it to the last section — thank you for being so thorough. This is where I can actually reach you.

Double-check this — it's where I'll send your confirmation.
